About Sage
Sun-Hee Yoon, Founder & CEO
Sun-Hee founded Sage Lifestyle Concierge in 2022 as an independent navigation firm — not a healthcare provider, not a placement agency — and has spent four years building deep working knowledge across the legal, financial, healthcare, insurance, real estate, and housing systems families must cross. She is a Dementia Connection Specialist (DCS) and serves on the Illinois Family Caregiver Coalition Advocacy Task Force, the Community Engagement Council at Rush Oak Park Hospital, and the founding Community Health Worker Advisory Board at Triton College. She sits on the boards of the Oak Park River Forest Chamber of Commerce and Continuity of Care West Cook.
She hosts and produces the Sage Pathways Podcast, moderates the ULCC Caregiving and Aging Support Group, and designed the Charting Your Courses and AgeWell educational series. Before Sage, she owned and directed an Eye Level Learning Center — experience in running a service business built on patience and trust.
She is also a family caregiver and care decision-maker, which is why Sage is built around the questions families actually ask — and why the answer is never shaped by who pays a referral fee.
Why Sage exists
Families deserve one guide who answers only to them
Most help in aging and caregiving is paid for by someone other than the family: the senior living community that pays a placement fee, the agency that pays for referrals, the product that pays a commission. The advice is often sincere, but the family is never quite the client.
Sage was built to be the exception. The family pays Sage directly, Sage earns nothing from anyone it recommends, and every professional it introduces has been vetted first. The mission is to reimagine aging with a proactive caregiving strategy — planning upstream, with grace and gratitude, rather than reacting downstream in a crisis.
The Sage Advisory Council
The people who keep Sage honest, current, and useful.
Sun-Hee is a navigator, not a licensed advisor. The council brings the licensed and lived expertise — law, medicine, care management, community, and commerce — that shapes Sage's guidance and holds it to a professional standard. Council members serve as advisors; they are not paid for referrals and do not receive clients in exchange for their role.

01
Standing
Current licenses and certifications, verified with the issuing body; complaint and disciplinary history reviewed.
How we vet providers · The Trusted Alliance Network
We would not send you to anyone we have not vetted ourselves.
Each provider is scored on all five principles; a provider must clear a minimum total score to be introduced to any Sage family.
Scores draw on license verification, public records, direct interviews, and — above all — how the provider actually performed for families Sage has followed through with.
Membership is reviewed on an ongoing basis. A pattern of missed expectations removes a provider from the network.
Appearing on the Sage Pathways Podcast, sponsoring it, or serving on the Advisory Council does not place anyone in the network. Vetting is separate from everything else Sage does.
How the evaluation works
02
Competence
Depth in the specific work older adults and caregivers need — not a general practice that takes the occasional elder client.
03
Communication
Plain language, timely responses, and patience with a family under stress — observed, not promised.
The Trusted Alliance Network is the group of attorneys, advisors, clinicians, Medicare specialists, realtors, and care providers Sage introduces families to. Membership is earned through a documented evaluation, reviewed over time, and can be lost. No one pays to be in it.
What it means for your family
You usually meet more than one option, you may bring your own professional, and you make the choice.
Sage prepares you for each meeting, attends when you want us there, and stays involved afterward to make sure the service delivered matches what was promised. If it does not, we help you say so — or help you change course.
04
Transparency
Fees and scope stated clearly up front; no hidden referral arrangements with other providers.
05
Integrity
The family's interest comes first — in how they advise, how they bill, and how they behave when no one is watching.
